Purpose of Collecting and Holding Information

As part of providing psychological services, the psychologist will collect and record personal and health information relevant to the client’s assessment, diagnosis, and treatment. This information is recorded as part of clinical case notes and helps ensure that appropriate, informed, and effective psychological care is provided. All information is stored securely in accordance with relevant privacy legislation and professional guidelines.

Access to Client Information

Clients are entitled to request access to information held about them in their clinical file. Access will be provided unless relevant legislation or professional guidelines restrict access (for example, where access may pose a risk of harm to the client or another person). Requests for access to records can be made in writing.

Release of Information

In providing treatment, the psychologist may need to obtain or share relevant information with other professionals involved in the client’s care (for example, GPs, paediatricians, schools, or other allied health professionals). Information will only be shared with the client’s consent, unless disclosure is otherwise required or authorised by law.

Confidentiality

All personal information gathered during the provision of psychological services will remain confidential and securely stored, except where:

  2. Failure to disclose the information would place the client or another person at serious and imminent risk of harm;

  3. The client (or parent/guardian where appropriate) has provided consent to: provide a written report to another professional or agency (e.g., GP, school, lawyer); or to discuss relevant information with another person involved in the client’s care;

  4. Disclosure is otherwise required or authorised by law

Use of Digital Documentation Tools

To assist with accurate clinical documentation, digital transcription tools may occasionally be used during sessions to support the psychologist’s preparation of written case notes. These tools convert spoken information into written text to assist with documentation. The purpose of this technology is solely to support clinical note-taking. Audio recordings are not stored, and all information is handled in accordance with privacy and confidentiality requirements. Clients are welcome to discuss or decline the use of this technology at any time.
